Definitely watch the first three seasons before you watch the fourth. I know some people start later in the show, which is a bad idea for GoT. For some shows it's okay, but you will be lost either way.

I wasn't really surprised with the amount of people, after everything leading up to that point, you kinda expect a shittonne of people to die. It's always who they kill that fucking surprises me.
