I have died plenty of times with a red skull, one time was in the middle of an open battle in 7.4. I lost my entire full set. :(

Many times! I sit there and tell myself "I just wasted so much freaking time hunting and now I am less of a level than when I started".

The day I first looted a set that worth more than 1kk on a red skull trap (demon helmet, fireborn giant armor, dwarven legs, 2 softs, boh, emerald sword, mms, 500k and avenger on backpack) or the day I first looted a DSM from a dragon lord, that day was awesome: 150k more on balance :D

Not in todays Tibia, but in the olden days it happened to me. I logged into a full spawn and was trying to spell out healing spells, while attacking while opening my backpacks to find my manas to use them and my UH's, unfortunately the few times it happened I couldn't get to my UH's fast enough!

I got killed, at a non Pvp server by a hunter. Inmediatly, I started to send messages to my Irl friends that invited me to the game (Noobiers as me), and we made a gang of 3 lvl 11, ready enough to get rid of the PK called Hunter.
We ended all dead that night.

Warskate Archer, he killed me when I was level 50... he was level 80. Soon after that I leveled up past him and was around 140 or so. At the time was second top level on the server. Warskate however was level 100. I pk'd him 2 times about an hour apart, he had totally forgotten about me. Ended up becoming one of my best friends. Good times lol.
